What are SOPs for an emergency evacuation for the elderly and passengers with reduced mobility?

The not too recent test evacuation of the A380 prompted me to write this. Perhaps someone from airlines and cruise operators could enlighten me on how operators deal with passengers that have reduced mobility (eg. wheelchair-bound, elderly, crutches, etc.) in terms of placement in the vessel (for normal operations). And also how these passengers are evacuated in the event of an emergency. Many thanks. No names of operators required.

How do you transfer a person safely from a wheelchair to a bed cot in an emergency shelter?

A movie demonstration or illustrated pictures would be good.
The cot is so low and of light structure. In an emergency evacuation to a shelter this can be dangerous for workers and residents.
